Job description

The Health Case Management Service provides a case management function for the population of Leeds (18 years and above) who are eligible for NHS Fast Track and Continuing Healthcare funding. Patients can be in the community or requiring support from acute settings. The service operates 365 days a year, over 7 days, Monday to Friday 08:00 to 18:00 and Saturday and Sunday 09:00 to 17:00, Bank Holiday hours 09:00 to 17:00.
