Job description

Job summary The aim of the role is to ensure that all patients are tracked through their ward stay, to ensure timely discharge including highlighting any escalations where necessary. The post holder will have awareness of and know how escalate delays along a patient's journey, working with the ward team, Integrated Discharge Team and Bronze on Call as required to minimise these.
