Job summary The post holder would be expected to champion a frailty approach to care and use comprehensive geriatric assessment to achieve the best care possible outcomes for older adults living with frailty in the Trust. They would be responsible for the care of 12-14 patients on a 25 bedded, acute delirium and dementia ward.
