Job summary This exciting role will be based in the Chesterfield and Dronfield Integrated Community Team. We have two posts available both 30 hours per week. This will be 4 days over a 7 day service. One post will be based at Ashgreen in the Newbold Team and the Other based at Rectory Road Surgery in Staveley.
