Job summary The Senior Clinical Decision Maker (SCDM) will provide advanced clinical oversight, decision-making, and coordination within the Reactive Care Coordination Hub. They will support timely responses to community-based crises, working collaboratively across UCR, Virtual Ward, and other teams to prevent unnecessary hospital admissions.
