Job summary An exciting opportunity has arisen within the Older Adult Mental Health Community services, where we are looking to recruit band 6 CPN to the WestHub Community Team. The post holder will care co-ordinate a caseload of patients with complex mental health problems, and complete initial assessments within Older Adult Services.
